Medical

Infertility Portal is pleased to offer you Everything Medical.  The journey through fertility can be overwhelming and that is especially true when it comes to the medical options available as well as how to choose an infertility doctor.  InfertilityPortal is here to try to simplify your options and give you the knowledge to choose the right IVF doctor and the treatment plan that is right for you.

If you are just beginning your fertility journey and do not  have an IVF doctor, you have come to the right place.  We offer you a unique platform where you can learn about how to select an IVF doctor and what to expect from the process.

If you have already begun IVF treatment – but you would like a second opinion about your current treatment plan , Infertility Portal offers you the opportunity to upload your medical records, complete a comprehensive medical questionnaire and consider another doctor or doctors to  review your treatment plan.

After years of fertility treatment some intended parents are nudged onto the path of Third Party Reproduction.  Essentially this is when a “third party” such as an egg donor, sperm donor, gestational carrier or a combination of all or several of these options.  However, how do you know when to switch to Third Party Reproduction and to which of the options?  Infertility Portal offers a wide range of articles, videos and webinars to help  you decide when and which option to select.  If you still need guidance, we encourage you to reach out to a mental health professional to discuss the pros and cons of each pathway towards parenting.  There is also the opportunity to seek a second medical opinion before you commit to a pathway.

The medical professionals  will need information from you in order to assist you to their fullest ability.  Below is a list of medical forms that you can peruse before committing to any payment.  These forms are lengthy, but are absolutely necessary before a medical professional can render an opinion.  Please note that some questions are asked for liability reasons, laws of some States/Provinces/Territories and/or countries .  We have endeavored to keep these forms to the least possible invasive questions.

Sample Medical Questionnaires

Please Note that these Medical Questionnaires are viewable only and cannot be completed or submitted.  These forms are shared with you so you can understand the kind of information that will be requested from you before you submit your medical records and/or request a second opinion on your present medical treatment plan or a second opinion on your future pathway to parenting.
